Health assessment is a significant part of diagnosis and treatment processes since it provides healthcare providers with patients’ health-related information needed for major clinical decisions. The most significant strength in this area involves the ability to listen attentively to patients without interrupting. Consequently, patients provide vital health-related information, which is used in making a differential diagnosis and developing a treatment plan.

However, an opportunity for growth concerning gathering patient information involves documentation of patients‘ health-related data. I tend to listen to a client without recording all provided information claiming I would easily remember it. As a result, significant data is inaccessible if the client is transferred to another healthcare provider or during future clinical, compromising the quality of healthcare provided to the patient. Thus, improvement is needed for better results during the next client encounter. I will practice how to document patients’ health-related information in electronic healthcare records (EHRs). According to Atasoy et al. (2019), all healthcare organizations operating in the US are required to implement EHRs. These systems are used for the documentation of patient information in EHRs, making patient healthcare data easily transferable or accessible while making a significant clinical decision, resulting in quality patient care and positive health outcomes.
